Multiple Offender Evaluations



The Georgia Department of Human Resources, along with the Georgia Department of Driver Services, requires anyone who is convicted of two DUI’s in a five-year period or a ten-year period to go through certain steps prior to reinstatement of a driver’s license.


    First, the offender must attend a Risk Reduction Program, commonly known as “DUI School.”  Once completing DUI School, the offender must request that the DUI School send a copy of the “NEEDS” assessment to the clinical evaluator.  This can be faxed to us at 770/898-8335.



    Second, the offender must obtain a “Clinical Evaluation” from an evaluator trained and certified by Georgia DHR.



    Third, the offender must complete any treatment recommended by the Clinical Evaluator


    Fourth, the offender must have an “interlock” device installed in any vehicle driven by the offender.  (We recommend Guardian Interlock in Riverdale, phone 770/478-8373.  We have found them to be honest and competent.)

Anger Evaluations



An arrest for simple assault or domestic violence frequently results in a referral by a court or probation officer for an evaluation to determine if there are any contributing causes to the incident.  A personal interview with the client, the victim (if possible), and family members, along with administration of appropriate assessment instruments can often shed light on the situation.  The evaluator can make recommendations for any appropriate treatment or other resources which could be helpful in preventing future occurrences. 



What Should I Bring to a Substance Abuse Evaluation?



• 7-year motor vehicle record • Dates of all lifetime DUI’s • Dates of all prior alcohol and drug treatments (if applicable) and your written discharge summary from all • Medical history, including current prescription names, dosages, prescribing doctor’s name, address and phone number • Employment history • Mental health counseling history • The “NEEDS” assessment should be sent by the DUI School prior to the first appointment.

What is an Alcohol and Drug Evaluation?



Most people who seek evaluations for substance abuse do so to comply with a Court Order or upon the recommendation of their attorney prior to court appearance.  An evaluation consists of a verbal interview, combined with one or more written tests.  When you arrive at the evaluation appointment, you will be given a personal history form to complete and the written test(s) will be administered.  You will then sit down with an evaluator for a personal interview, which will include your medical, family, educational, legal and employment history.  The interviewer will also ask you about any previous treatment or counseling.  The entire process can last up to two hours.
